I agree that I wish to receive communications and/or information regarding my pet(s) health, medical condition, arranging for medical treatment or advice, or setting up an appointment or home visit from Mirthwood Medicine, including by email, phone, mail and transactional SMS Text Messaging using the contact information I am submitting.

If you ever wish to stop receiving text messages from us, simply respond ‘STOP’

I consent to Mirthwood Medicine and its service providers processing my personal data for these purposes. I understand that I can withdraw my consent at any time

I confirm that I am 18 years or older and I consent to Mirthwood Medicine using my personal data in order to discuss my Pet’s medical issues and/or request an appointment with their veterinarians or assistants.

 

Terms of Service for Veterinary Care & Treatment:

 

Authorizations:

I hereby authorize the veterinarians at Mirthwood Medicine and their staff to examine and/or perform the necessary treatments or euthanasia on my pet(s) as deemed necessary. If any unforeseen condition arises during the medical procedures or euthanasia calling for their judgment for any procedure in addition to or different from those now contemplated, I further authorize them to do whatever is necessary to avoid unnecessary suffering by the animal.

I acknowledge that no surgical or anesthetic procedure is without some risk to the animal. I accept all potential procedural, surgical, and anesthetic risks including any complications that may develop as a result of this procedure and accept that such complications may incur additional fees.

I agree to assume financial responsibilities for services provided. I acknowledge that no guarantee has been made as to the results that may be obtained from procedures or therapies.

I hereby authorize the veterinarian(s) and/or their assistants of Mirthwood Medicine to provide such additional medical or surgical services for my pet as they may deem necessary, including, but not limited to, the administration and maintenance of sedation or anesthesia, and the performance of any services required. I understand that medical records will not be disclosed to anyone other than myself, my agent, or my referring veterinarian without my permission.

I understand that my veterinarian’s goal is to report lab and diagnostic reports pertaining to my pet as soon as possible. However, if I do not hear from my veterinarian or Mirthwood Medicine staff within the time specified, I will call the office for the test results.

I am the legal owner or the representative of the legal owner of the animal being presented, and I am over the age of 18 years. I understand and agree with the terms of this policy.

 

Force Majeure

Mirthwood Medicine shall be excused from any delay or failure in performance required hereunder if caused by reason of any occurrence or contingency beyond its reasonable control, including, but not limited to, acts of God, acts of war, fire, insurrection, strikes, lock-outs or other serious labor disputes, riots, earthquakes, floods, explosions or other acts of nature.

 

Limitation of Liability

The Client acknowledges and agrees that Mirthwood Medicine will not be liable for any losses or damages, whether indirect, incidental, special or consequential, in profits, goods or services, irrespective of whether or not the Client has been advised or otherwise might have anticipated the possibility of such loss or damage.

 

No Guarantee

The client acknowledges and agrees that Mirthwood Medicine cannot guarantee the results or effectiveness of any of the services rendered or to be rendered. Rather, services shall be executed in a professional manner and in accordance with good veterinary medical practice. Best efforts will be used but no results are promised.
